SAME DAY SHIPPING
    Most orders received before 12:00 Noon CST will be shipped the same day.

ONLINE ORDERS
    - Once an order has been placed through our website, an automated email will be sent
       with a copy of your order and an Order Status Link. (Please check your order for accuracy)
    - When your order has shipped, a similar email stating your order has shipped will be
       sent with a copy of your order and an Order Status Link which may be used to obtain
       your tracking number(s), order total and COD amount (if any).
    - When your order has shipped through UPS, an automated email will be sent by QuantumView
       UPS Ship Notification with a tracking number and link to track your package(s).

When placing orders online, please add our email addresses (sales@danchase.com &
danchase@danchase.com) to your email address book to help prevent Email Blocking
Programs from blocking your order notifications.


CREDIT CARD ORDERS
    We accept Mastercard, Visa and Discover

COD ORDERS  (Collect on Delivery)
   - All COD orders will have a $2.00 COD fee.
   - Orders shipped UPS COD have an additional $7.50 COD fee.
   - Orders shipped USPS COD have an additional $4.50 and up COD fee. [see chart]
 

- COD orders may be required to be paid by Money Order/Cashier's Check,
   if so an attempt will be made to call with a COD total once the order has shipped.
        [online orders can also be checked through the link received by email]
- Payment by Money Order/Cashier's Check will be required until $300 worth of supplies
   have been purchased.
- Once the $300 minimum has been reached, COD's can be payable by company or personal check.
- If a Check has been returned NSF, etc., COD's will be returned to Money Order/Cashier's Check.

    - UPS will make three (3) delivery attempts to collect COD orders.

MINIMUM ORDERS
    Orders under $15.00 will be subject to a $2.00 handling fee.

BOXING CHARGE
    Orders containing less than 3 head forms & less than $60, will be charged a $2.00 fee.

N.S.F. CHECKS
    All checks returned NSF, Account Closed or for any other reason will have a $20.00 handling
    charge and a $5.00 bank fee charge.

ADDRESS CORRECTION FEE
    Any package(s) delivered to an address different from the address shipped to
    will be subject to a $5.00 fee.

CUT FEE
    Some extra large forms may be subject to a $25.00 cut fee if shipped UPS instead of TRUCK.

    Also, most of these extra large forms will need to have the legs cut off at the body to be able to ship UPS.


SHIPPING

Customers are responsible for all shipping charges.
This also includes packages that are not delivered and returned to us due to:
Wrong Address, Does Not Want, Refused, Not in on 3 Delivery Attempts, No Funds Available or any other reason.

UPS - United Parcel Service        
   - All orders are shipped UPS daily, unless otherwise requested.
   - All orders are shipped UPS Ground Residential.
   - Next Day Air, Second Day Air & 3 Day Select services are also available.
        UPS Air Services can be very expensive. All UPS Air Package rates are charged
        by dimensional weight. [To calculate dimensional weight multiply
        Length X Width X Height and divide by 194. This is the dimensional weight.]
  - UPS orders can not be shipped to P.O. Boxes.
  - Packages can be tracked at www.ups.com

USPS - United States Postal Service       
   - Orders requesting USPS will be mailed Priority Mail. Air Mail can be requested.
   - Orders requesting USPS are shipped approximately every other week.
   - Packages can be tracked at www.usps.com

TRUCK - Usually Roadway Express
   - All orders shipped by Truck will have a $10.00 boxing charge.
   - Shipping charges will be paid directly to the trucking company upon delivery.
   - This is usually to most economical way to ship extra large lifesize forms.

LOCAL PICKUP
   - Orders must be called in before 12:00 noon CST to be able to pickup the same day.
   - Orders can then be picked up after 2:00pm the same day.
   - Orders placed after 12:00 noon CST can be picked up after 2:00pm the following day.
   - ORDERS CAN NOT BE PICKED UP BETWEEN 11:00am and 1:00PM

DROP SHIP
   - Drop Shipped item(s) are shipped directly from the manufacturer.
   - Item(s) will be shipped by trucking company and shipping charges will be paid
     directly to the trucking company upon delivery.



RETURNS
All returns/exchanges must be made within 30 days of the invoice date provided that the
merchandise is in good saleable condition. CUT FORMS ARE NON-RETURNABLE. Any returns/exchanges
older than 30 days must be pre-approved and will be subject to a 15% restocking fee. Please
include a copy of the invoice. Without proper information we can not process your return. Video
tapes and DVDs may only be returned for the same Video or DVD number. (VHS can be exchanged for
same video in DVD and vice versa)


DAMAGES/DISCREPANCIES
Please check in all shipments upon receipt and notify us of any discrepancies or damages
immediately. Notifications received after 10 days may not be honored.

Damaged forms can be replaced or given a credit for repair costs.
